Four Easy Ways to Enroll in Bravo Health

1. Over the Phone

 

Call 1-800-821-7513 Seven days a week from 8 am to 8 pm.
TTY/TDD users, please call 711.

2. Online


Medicare beneficiaries may enroll in Bravo Health through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at http://www.medicare.gov/ or call 1-800-MEDICARE (1-800-633-4227), 24 hours a day/7days a week. TTY/TDD users should call 1-800-325-0778.

For more information, contact Bravo Health at 1-866-442-7498. Customer Service hours of operation are 8 am to 8 pm, 7 days a week. Please call TTY 711 for the hearing impaired.

3. Through the Mail

Call 1-800-821-7513 seven days a week from 8 am to 8 pm, and one of our Sales Representatives will be happy to mail you a free enrollment kit.  TTY/TDD users, please call 711.
