• Beneficiary's date of birth

• Current address

• Phone number

• Beneficiary’s Medicare card & number

• Medicare plan enrollment

• Other insurance plans and policy numbers if applicable

• Current medical providers

• Preferred hospitals

• Current list of prescription drugs and dosages

• Does your loved one want you to become an appointed representative?

• Current medical insurance including Medicaid if applicable
